Nominations must be completed using the form below. Nominations can be made by your company, your manager, your colleague, or you can self-nominate.

Nominations are open to all persons who qualify under the awards category criteria.

Please ensure you have completed ALL sections in the form as incomplete nominations cannot be processed.

Use one nomination per form submission. After your submission you can refresh the page to submit another nomination.

Include relevant supporting evidence that highlight key points of difference and shows how the nominee stands out from their peers. During judging the nominator could be contacted to get clarification or more information.
Provide a brief overview with consideration as to why the nominated individual/company/crew deserves this award. If an individual, comment on character traits inside and outside of the workplace.
